Requirements By Grade:

• PreK: Physical Exam, Immunization Records, Lead Screening

• Kindergarten: Physical Exam, Immunization Records, Lead Screening, Vision Exam, Dental Exam

• 2nd Grade: Dental Exam

• 6th Grade: Physical Exam, Tdap, Meningococcal Vaccine, Dental Exam

• 9th Grade: Physical Exam

• 12th Grade:  Meningococcal Vaccine

• Transfer Students from Another State: Physical Exam, Immunization Record, Vision Exam

Sports physicals are recquired annually for all students participating in school sponsored sports.
